Based on record review at 9:15 a.m., S1 failed to have written authorizations, signed and dated by the parent, to release 7 out of 10, C3 (5-years-old, C9 (6-years-old), C4, C8, and C10 (8-years-old), C7 (9-years-old), and C5 (11-years-old), to the school bus. This was not corrected during the inspection. Corrective Action: Effective 08/18/2015, S1 created a written authorization form including transportation services and will have all enrolled children complete by 08/22/2025, to ensure compliance with this regulation.
