      During the inspection, it was determined that a nap/rest period was not reflected in the daily schedule for children attending more than 5 hours per day.

      During the inspection and review of staff files, all staff onsite did not have a current age-appropriate course in CPR from an approved provider. In addition to submitting a corrective response in OCLQS, a copy of the training documentation must be e-mailed to Margaret.Agoston@childrenandyouth.ohio.gov on/before the corrective action plan deadline. The documentation must include training card(s), form(s), certificate(s), and/or OPR verification for all staff employed more than 90 days.
