      1507.B.3 Based on record review and interviews at 12:00 p.m., S1 failed to ensure the center's staff daily attendance record accurately reflect persons on the child care premises at any given time on 4/17/2026. S1 stated she left the center to run errands around 10:00 a.m. and returned at 11:00 a.m. The staff attendance shows S1 at the center from 6:45 a.m. to 5:37 p.m. Corrective Action: Effective 05/04/2026, S1 stated she will sign in and out every time she leaves the center to ensure compliance with this regulation.

      Based on interviews and record review at 2:15 p.m., S1 failed to ensure that incidents, injuries, accidents, illnesses, and unusual behavior were documented and reported to the parent immediately. On 5/28/2025 at approximately 9:00 a.m., C1, 2-years-old, fell from the changing table and hit his head on the floor while S17 was preventing another child from climbing onto the changing table. Based on record review and interviews at 10:58 a.m., S1 failed to immediately notify the parents of the following critical incident: On 3/24/2025 at 9:02 a.m., C1, age 2-years-old, was left alone on the large outside play yard for 55 seconds by S3. S3 retrieved C1 at 9:03. O1 and O2 were notified of the critical incident on 03/24/2025 at 6:55 p.m. Corrective Action: Effective 04/15/2025, S1 stated she will remind staff to immediately notify her of critical incidents so that she can immediately notify parents to ensure compliance with the regulation.
