Based on observations at 11:15 a.m., the provider failed to have the outdoor area free of hazards. A pile of pvc pipes were accessible to children on the playground. Corrective Action: Effective 4/20/2026, S1 stated she will request maintenance remove the pipes to ensure compliance with this regulation

Based on record review and interview(s) at 11:45 a.m., S1 failed to ensure C1, age 2-years-old, was not served restricted foods identified by O1. C1 is allergic to milk and on 4/7/2026, S1 gave C1 whole milk for lunch. C1 had an allergic reaction that required the administration of emergency medication and medical attention. Corrective Action: Effective 4/20/2026, S1 stated children with milk allergies will be served milk in a colored cup to ensure compliance with this regulation.
