      The following deficiencies were observed at the time of the Initial Compliance inspection conducted on 12/17/2025, and are subject to change pending programmatic review. A paper copy of the Notice of Inspection Rights was offered at the time of the inspection. Fifteen staff files were reviewed, and all fingerprint clearance cards were verified as valid via the DPS website. The following was discussed, but not limited to: *Completed children's medication consent forms; Licensure is
