Lead Teacher put a partial, opened bag of marshmallows on the diaper changing table. The table still had a bottom barrier on the changing pad. When asked, provider stated she had not removed it from the last diaper changed. Surveyor had been in the room 20 minutes and had not seen any of the children have their diaper changed.
