Based on observation/record review/interview at 10:15 a.m., S1 failed to have a qualified Director or Director Designee who is an on-site full time staff person at the center during the day time hours of operation (prior to 9:00 p.m.) and responsible for planning, managing, and controlling the center's daily activities, as well as responding to parental concerns and ensuring that minimum licensing requirements are met. Based on record review at 10:15 a.m., S1 failed to have documentation that S5 completed the online child abuse and neglect Mandated Reporter Training provided by DCFS. S5's last training was expired on 2/24/2025. This was not corrected prior to the Specialist departure. Corrective Action: Effective 8/25/2025, S1 stated she will make sure S5 completes the Mandated Reporter Training by 8/29/2025. She will set up a tracking system to track the trainings to make sure the trainings do not expire to ensure compliance with this regulation.
