      Prevention of child maltreatment- A provider used inappropriate discipline with a child in care. All providers must use discipline methods as outlined in ARSD 67:42:17:41. The provider involved in the incident is no longer employed with the program and the Office of Licensing and Accreditation (OLA) will continue to monitor the program through the corrective action plan that is currently in place.

      Sanitation requirements- A provider was observed failing to wash their hands with soap after changing diapers. All providers, program employees, and children shall wash their hands with soap, before preparing food or beverages, eating, handling food, or feeding a child, and after changing a diaper, using the toilet, helping a child use a toilet, or coming into contact with bodily fluid. The Office of Licensing and Accreditation (OLA) will require the provider, who failed to wash their hands after changing a diaper, to complete training on proper sanitation practices. Additionally, the sanitation and handwashing procedures were reviewed with providers.

      Medication administration- There was numerous medications at the program that were no longer needed or expired. Before any medication is administered to a child, permission of the parent or guardian must be documented and must include the name of the child, the name of the medication, and the dates, times, and dosage of the medication. The medication must be returned to the parent when no longer needed or expired. All medication that is no longer needed or expired was returned to parents. Medication authorization forms were obtained from parents for the medication that was kept at the program.

The program has developed and implemented a written plan to ensure the staff to child ratio is maintained at all times. The Office of Licensing and Accreditation (OLA) will conduct monitoring visits to ensure ratio is maintained.

A Corrective Action Plan has been implemented with the program and monitoring visits will be conducted by the Office of Licensing & Accreditation for a period of 60 days.
