      1 child's file missing the correct documentation for a birth certificate. Send documentation to consultant.

      Poisons, chemicals, and hazardous items In the Fawn room there was Dawn dish soap and Johnsons baby shampoo both labeled Keep out of Reach of Children under the sink behind a child deterrent.

    • Staff Shall Offer an Additional Portion of Food and Juice or Milk at Snacks

      At snack time in the Grizzly room there was no additional snacks available to offer a child who asked for seconds.

    • Bathrooms

      In the bathroom of the Fawn room there were clothes and a backpack stored open, not sanitary. In the Grizzly bathroom there were towels and a playmat stored open in the bathroom, not sanitary. In the Grizzly and Owls/Eagles bathrooms there was one toilet in each without toilet paper on the dispenser
