Based on observations and interviews on 02/26/2026 at 12:00 p.m., S1 failed to ensure that children were supervised during rest time. On 02/26/2026, at around 12:01 p.m., while the Specialist was walking around the center, it was observed that six, 1-year-olds were left unsupervised in the classroom with no staff during nap. The children remained unsupervised for approximately 4 minutes until S1 returned to the classroom with the 1-year-olds. S1 stated she was in the neighboring classroom and thought as long as she was in proximity of the sleeping children, they were supervised.…

1727.A.&B.: Based on record review/interview at 12:00 p.m., S1 failed to ensure all staff members completed the Mandated Reporter training annually. 2 of 8 staff members, S1, failed to have a current copy of S1 and S2’s Mandated Reporter training on file. S1 and S2’s Mandated Reporter training expired on 03/24/2025. S1 stated she thought they had completed the training, but could not locate a current certificate. Corrective Action: Effective 02/26/2026, S1 stated all staff will complete the Mandated Reporting Training annually going forward, and when complete will place their certificates…

1711.A. B. D.: Based on observation/interviews at 9:00 a.m., S9 failed to meet the required child to staff ratio for children of the following ages: 28, two to five-year-olds, were being supervised by one staff member, S11 . The required ratio for two to five-year-old is 14:1. Corrective Action: Effective 5/28/2025, S9 stated they will ensure child to staff ratios are met at all times by having an additional staff member at the center as needed. Interviews with prospective employees have been scheduled for 5/28-29/2025, to ensure compliance with this regulation.

1103.A.5.C.1. Based on record review/interview(s) at 11:30 a.m., S1 and S3 failed to notify the Department within 24 hours of the following critical incident: On 2/12/2025 at 3:05 p.m., O4 conducted an investigation at the center of the following allegation of suspected abuse/neglect of C1, two-years-old. O1 alleged the center staff of abusing C1 while in the centers care. S1 stated she did not report the visit from Child Welfare, because she was not aware DCFS visits had to be reported to the Department. The Specialist was unable to validate the complaint.

1711.A. B. D.: Based on observation/interviews at 8:15 a.m., S3 failed to meet the required child to staff ratio for children of the following ages: 19, three to four-year-olds, were being supervised by one staff member, S4, 7, infant to one-year-olds, were being supervised by one staff member, S9, and 6 infants, were being supervised by one staff member, S10. The required ratio for infants to one-year-old is 5:1, and children three to four-year-old is 14:1.

Based on observations/record review, at 2:30 p.m., S5 (DOH: 2/07/2025) and S6 (DOH: 1/24/2025), failed to have current certification in CPR prior to assuming sole responsibility of children. The Specialist observed S5 alone supervising 8, two-year-olds, and S6 alone supervising 9, two-year-olds. S2 (DOH 2/15/2018), S5 (DOH 2/07/2025), and S6 (D0H 1/24/2025) failed to have current certification in CPR. A CPR training has been scheduled for 2/22/2025 with an approved trainer.

Based on observations/record review, at 2:30 p.m., S5 (DOH: 2/07/2025) and S6 (DOH: 1/24/2025), failed to have current certification in Pediatric First Aid prior to assuming sole responsibility of children. The Specialist observed S5 alone supervising 8, two-year-olds, and S6 alone supervising 9, two-year-olds. S2 (DOH 2/15/2018), S5 (DOH 2/07/2025), and S6 (D0H 1/24/2025) failed to have current certification in Pediatric First Aid. A Pediatric First Aid training has been scheduled for 2/22/2025 with an approved trainer.

1509.A.1. Mandated Reporter: Based on record review/interview at 12:00 p.m., S1 failed to ensure all staff members completed the Mandated Reporter training annually. 1 of 13 staff members did not have current certification.

: Based on observations at 12:00 p.m., S1 failed to ensure that the all cribs were free of toys and other soft or loose bedding, including comforters, blankets, sheets, bumper pads, pillows, stuffed animals and wedges when a child is asleep in the crib. The Specialist observed one infant child, C1 (5 months old), laying down with a portable music speaker and a small toy next to the child while sleeping.

: Based on observations at 12:00 p.m., S1 failed to ensure that all infants were held while being fed. The Specialist observed one infant, C2 (9 Months), in the crib laying down flat with a bottle next to them.
