      Diapering requirements were not met as evidenced by the diapering table was not located where caregiver could maintain supervision of his/her group of children. The diapering table was located against the wall, and the caregiver would have their backs to the group in both of the infant/toddler room

      Outdoor equipment, climber with slide and rocking horse, in the play yard was not free of sharp parts as evidenced by the plastic climbing handles on the climber/slide are broken and the plastic rocking horse has a hole in it's head that is sharp.
