      1103.C.1.Based on record review and interview at 1:00 p.m. 2/23/2026, S1 failed to notify LDOE within 24 hours following critical incident: On Thursday, 2/5/2026 at approximately 9:30 a.m., S1 observed S3 drape her legs over the back of C1, four-years-old, as he lay on his cot during naptime. S1 made the report to LDOE on Monday, 2/7/2026 after law enforcement reported to the center. Corrective Action: Effective 2/23/2026 S1 stated she will retrain all of her staff on mandated reporting to ensure compliance with this regulation.

      1507.E. Based on record review on 2/23/2026 at 11:30 a.m., S1 failed to maintain documentation of a daily attendance record for visitors to include arrival and departure times, and purpose of the visit for a Washington Parish Sheriff’s Officer. Corrective Action: 2/23/2026, S1 stated she will check the daily attendance daily to ensure visitors are signing in and out to ensure compliance with this regulation.

Based on interviews on 2/23/2026 at 12:00 p.m., although there is a behavioral management policy implemented, S3(DOH 1/29/2024) used a prohibited method of discipline as a child was subjected to physical punishment. On 1/29/2026 at 11:25 a.m., S3 draped her legs over the back and legs of C1, four-years-old, while he laid on his cot during naptime to get C1 to go to sleep. Corrective Action: Effective 2/23/2026, S1 stated she will remind staff of the behavior management policy to ensure compliance with this regulation.
