Commonwealth Child Care Credential as described in 922 KAR 2:250; Findings: General: Based on review of documentation, one staff member (DOH:12/16/25) did not have educational documentation

Findings: General: Based on observation, the surveyor found multiple brown circular stains on the ceiling tile next to the wall in the downstairs bathroom. Below the middle tile, there were brown lines coming down from the tile to the wall; therefore, the child-care center failed to keep the ceiling and walls in good repair.
