      Learned through investigation that a small rock approximately one half (1/2) inch to one (1) inch in diameter was mixed in with the resilient material wood chips directly below one (1) of the swings in the outdoor play area. Child1 hit their head on the rock that was lying in the resilient material wood chips while utilizing the swings.

      The wood chip resilient material measured at a depth of three and a half (3 1/2) inches in the use zone below the Red slide, the red, blue, and green climber, and the, swings that are part of the outdoor play area equipment.

      In the playground near the North and middle cottage the wood chip resilient material in the use zone at the bottom of the red slide measured five inches (5") in depth. The wood chips under the blue, red, and green climibing structure measured four inches (4").
