      On 6/13/2024, children were not within sight or sound during naptime. Children must be within sight or sound at all times. The licensee had since modified the house by removing a wall to increase visability.

      The caregiver shall report immediately suspected physical abuse, sexual abuse, c The childcare did not immediately contact (within 24hours) CPS or law enforcement agency (LEA). A CPS report was made on 7/1/2024. However, Licensing and CPS/LEA must both be notified immediately.
