Identified through review of 4/16/25 video footage of the Toddler classroom, SM1 was working alone in the classroom with four children. While SM1 was changing a child's diaper, SM1's back was turned away from the three other children in the classroom. Identified through review of 4/17/25 documentation SM1 lacked "active supervision, including extended periods where SM1's attention was not on the children and SM1's back was turned to key areas of the classroom". This is a direct violation of Stipulation D on the Probationary License. Stipulation D states "All children must be under the direct and active supervision of qualified staff at all times, both indoors and outside".
