Based on record review and interviews at 12 p.m., S1 failed to notify DCFS within 24 hours of the following critical incident: On 5/13/2025, around 1 p.m., C1, 3-years-old, was left outside on the play yard for approximately 5-10 minutes after the rest of the class went inside. C1 was found by the school's custodian and returned to S1. S1 submitted a CIR to the Department on 5/14/2025, but failed to report the incident to DCFS.
