A sample of children's emergency forms were reviewed and within this sample there are 3 forms that are missing the Physician's information. Please have the parent/guardian complete the required information on the emergency forms and send a copy to the licensing specialist. See additional comments

A sample of children files were reviewed and within this review there were 4 children records that needed updated immunizations and 1 child's record that did not have evidence of immunizations. The staff files were reviewed and there is one staff that does not have evidence that the center orientation was completed with them. Please complete the orientation with this staff person send a copy to the licensing specialist.

There are 4 children that do not have a written individualized activity plan; please have the staff complete a plan in consultation with the parent and post for reference. Be sure to have these updated every 3 months.
