      Based on observation at approximately 11:15 a.m., while supervising a group of children, childcare staff, S11, S12, and S13, failed to devote their time to supervision of the children, meeting the needs of the children, and in participation with the children in their activities; staff was observed seated on the floor of the multi-purpose room talking and utilizing their cell phones. S11, S12, and S13 all got up and began participating with the children prior the Specialist leaving the center. Corrective Action: Effective 10/29/2025, S1 stated she will complete a…

      Based on observation at approximately 11:20 a.m., the personal belongings of center S13 and S14's classroom there were two tumbler cups (red and white) and a pizza box in spaces accessible to children. S1 moved the personal items prior to the Specialist leaving the room. Corrective Action: Effective 10/29/2025, S1 stated she will re-train all staff on the importance of ensuring that staff personal items are not accessible to children at all time by 11/7/2025, to ensure compliance with this regulation.

      Based on record review at approximately 11:15 a.m., the indoor area failed to be free of hazards as S13 and S14's classroom had adult scissors, and hot glue guns in spaces that are accessible to children. S1 moved the adult scissors and glue gun out of the area prior the Specialist leaving the classroom. Corrective Action: Effective 10/29/2025, S1 stated she will re-train all staff on the importance of ensuring that license space is free of hazards at all time by 11/7/2025, to ensure compliance with this regulation.
