C.: Based on observations, record review, and interviews at 12:30 p.m., on 10/28/2024, although S1 notified the Department and Child Welfare within 24 hours of a critical incident involving C1, 3-years-old, S1 failed to notify the parent immediately. Based on the Critical Incident Report, at 11:25 a.m., 10/22/2024, S2 failed to know C1's whereabouts at all times as C1 went unnoticed into S4 and S5's classroom instead of the restroom and was there for 5-10 minutes, while S1 and S3 looked for her.

Based on observations, record review, and interviews at 12:30 p.m., on 10/28/2024, S2 failed to know the whereabouts of all the children assigned to her as at 11:25 a.m., on 10/22/2024, she failed to notice C1, 3-years-old, go into S4 and S5's classroom instead of the restroom and stay there for 5-10 minutes. C1 was in the classroom for 5-10 minutes while S1 and S3 looked throughout the center and on the playground for her.
