Based on record review at 12:00 p.m., S1 failed to have documentation for 1 of 8 staff members on the premises and accessible to children had current certification in adult, infant, and child CPR through training approved by the department. S3 failed to have the current certification. This was not corrected prior to the Specialistâs departure. Corrective Action: Effective: 10/30/2025, S1 stated she will have S3 complete an approved CPR training course, and she will create a calendar reminder for all staff training expirations to ensure compliance with this regulation.

Based on interviews and record review at 12:00 p.m., on 12/18/2024, although the center has implemented their written behavior management policy prohibiting physical punishment, S3 (DOH 08/22/2016) used a prohibited method of discipline on 12/11/2024. At 10:55 a.m., S3 was observed by S2 in the girls' restroom holding C1, 3-years-old, on her hip and hitting C1's leg. S3 was terminated on 12/11/2024.
