Based on record review/interviews on 11:30 p.m., although a mandated reporter, S20 (DOH 3/17/2026), failed to report abuse of a child to the Child Protection Statewide Hotline. On 5/7/2026, at 10:30 a.m., S20 observed S26 grab C1, age 2-years-old, by the hair to stop him from running in the indoor play area. Corrective Action:

Based on record review and interview at 12:15 p.m., although the center has a Behavior Management Policy implemented, S26 (DOH 10/14/2024), used a prohibited method of discipline as C1, age 2-years-old, was subject to physical discipline. On 5/7/2026, at 10:30 a.m., S26 grabbed C1 by the hair to stop him from running in the indoor play area. S26 was terminated from the center on 5/8/2026. Corrective Action: Corrective Action: Effective 5/21/2026, S1 stated she will conduct a training with staff on behavior management to ensure compliance with this regulation.

Based on record review and interview, on 04-08-2026 at 1:35 p.m., S1 failed to maintain documentation of a daily attendance record for S7, S14,and S16, to include the time of arrival and departure. S7, S14, and S16, were present during the licensing visit on 04-08-2028, and were not signed in or out on that date or any other dates. Corrective Action: Effective 04-08-2026, S1 will have maintain a sign-in and sign-out sheet for other staff not directly employed by the center, to ensure compliance with this regulation.

Corrective Action: Effective 04-09-2026, S1 stated she will place a student sign in log the office to ensure compliance with this regulation.

: Based on record review and interviews on 04-09-2026 at 12:52 p.m., S32 (DOH 12-03-2021) used a prohibited method of discipline as children were subject to verbal abuse. On 01-29-2026, O2 stated she heard S32 tell the children that she was with in the hallway to shut up. S32 was terminated on 01-29-2026. Corrective Action: Effective 04-10-2026, S3 stated staff will be retrained on the Behavior Management Policy and staff's tone will be monitored when with children to ensure compliance with this regulation.

1711.A.&B.&D: On 04-08-2026 at 11:04 a.m., S1 failed to meet the required child to staff ratio for 1-year-old-children. The Specialists observed S25 leave S24 in the classroom with nine 1 to 2-year-old children to retrieve lunch. The required ratio for children of this age is 7 children per 1 staff person. S24 returned to the classroom at 11:05 a.m. to satisfy ratio. Corrective Action: Effective 04-09-2026, S1 stated she will ensure all staff ratio is met at all times to ensure compliance with this regulation.
