The health plan from a health care provider was available for review.

      Nine (9) cans of shaving cream that stated "keep out of reach of children" were located at the bottom of the last white cabinet to the right in the row of cabinets located at the back of the cafeteria room. The cabinet was unlocked and the cans of shaving cream were accessible to children.
