Based on record review and interviews at 1:30 p.m., S1 failed to ensure children were under supervision at all times. On 8/25/2025, at 10:13 a.m., C1, 2-years-old, was left outside on the small playground for 2 minutes and 39 seconds by S5 and S8. C1 was retrieved by S5 at 10:15 a.m. Corrective Action: Effective 9/10/2025, S1 instructed staff to not allow more than 1 class at a time to re-enter the center from the playground. S1 also instructed staff to have 1 teacher take the children into the classroom and have a second teacher sweep the playground to make sure all children have…

1713.A.B.C. Based on record review/interview at 10:42 a.m., S1 failed to ensure children were supervised at all times. On 12/16/2024, at 9:17 a.m., C1, age 2-years-old, ran out the back door of Classroom 4 at 9:17 a.m. and onto the gated playground for approximately 2 minutes. S12 and S13, teachers, were in the classroom with the other students and did not notice C1 was gone. According to S1, S13 retrieved C1 from the outdoor playground at 9:19 a.m. when C1 knocked on the classroom door to be let back in.
