Based on observation at 10:56 a.m., S2 failed to meet the required child to staff ratio for children. The Specialist observed 6 children, ages 8-months-old through 1-year-old, in S3’s classroom. The required ratio for children of this age is 5 children per 1 staff person. One additional staff person was needed at all times to meet child to staff ratio. Ratio was corrected during the inspection. Corrective Action: Effective 5/22/2026, S2 will complete a walkthrough of all classrooms hourly to make sure ratio standards are met to ensure compliance with this regulation.

1907.A.1. Based on observation at 10:52 a.m., S5 and S6 failed to ensure the manufacturer's restraint device was in use when feeding seats were occupied by children. The Specialist observed C1, 4-year-old, and C2, 2-year-old, seated in the feeding chairs and the manufacturer's restraint device failed to be in use. This was corrected prior to the Specialist exiting the classroom. Corrective Action: Effective 5/22/2026, S2 will address with all staff that the manufacturer's restraint device must be used when occupied by children to ensure compliance with this regulation.

Based on record review at 1 p.m., S2 failed to ensure the center's daily attendance record for children accurately reflected the children on the child care premises at any given time, as 26 children were present and 30 children were signed in on the log. This was corrected during the inspection. Corrective Action: Effective 10/27/2025, S1 stated he will review this regulation with all staff, and S1 and S2 will conduct a review in the morning and afternoon to ensure the children are signed out appropriately, to ensure compliance with this regulation.

1711.A.B.D&E.: Based on observation at 1:08 p.m., S2 failed to ensure the required child-to-staff ratio was met for children of the following ages: 7 children aged infant to 1-year-old with 1 staff, S6. The required ratio for children of this age is 5 children per 1 staff person. The required child-to-staff ratio for children of this age is 5 children to 1 staff person. Based on observation at 1:09 p.m., S6 allowed C1, Infant, to lie down holding a bottle in the crib. S6 corrected by taking C1 into her lap to feed her prior to the Specialist leaving the classroom. Corrective Action: Effective 10/27/2025, S1 stated he will review this regulation with all infant staff, to ensure compliance with this regulation.

1507.A. Based on review of records at 11 a.m., the daily attendance log for children failed to include the first and last name of the person to whom the child was released. The Specialist reviewed records from 6/15/2025, until present, and verified the name of the person to whom the child was released failed to be documented 16 times. Corrective Action: Effective 6/30/2025, S1 will require the closing staff to review the children's attendance daily to make sure all children are signed out properly to ensure compliance with this regulation. 1907.E.2. Based on observation at 11:05 a.m., the center's cribs failed to be free of toys while a child was in a crib. The Specialist observed a toy in the crib with C1, 7-month-old. This was corrected prior to the Specialist exiting the classroom. Corrective Action: Effective 5/28/2025, S4 will review with infant room staff that all cribs must be free of toys to ensure complaince with this regulation.

1911.G. Based on observation at 11 a.m., a pacifier was attached to a child. The Specialist observed a pacifier attached the clothing of C3, age 1-year-old, in S6's classroom. This was corrected prior to the Specialist exiting the classroom. Corrective Action: Effective 5/28/2025, S4 will review with all staff that a pacifier cannot be attached to a child to ensure compliance with this regulation.

A.: Based on record review, at 9:30 a.m., the center's daily attendance record for children failed to include the time of arrival, time of departure, and the name of the person to whom the child was released. The Specialist reviewed children's attendance from 01/28/2025, until present, and verified the following: the arrival time failed to be documented 2 times; the departure time failed to be documented 57 times; the name of the person to whom the child was released failed to be documented 38 times.

1919.J. Based on observation, at 9:15 a.m., S5 failed to have bottle formula/breast milk for infants labeled with the child's name. The Specialist observed 6 of 12 bottles unlabeled. This issue was corrected prior to the Specialist to exiting the center.
