Based on record review on 01/21/2026, at 12:30 p.m., the center's Daily Attendance Children’s record shall be maintained to accurately include first and last name, arrival and departure times, date, and the last name of the person or entity to whom the child is released. To accurately reflect children on the center’s premises at any given time and be used to sign in and out if a child leaves and returns to the center during the day. Based on observation at 2:30 p.m., S6 failed to have a record of all staff members and owners, including the first and last name of the staff member or owner, date, and arrival and departure times, to accurately reflect the staff members and owners on the premises at any given time. S6 did not have access to files because Procare was not working. Corrective Action: Effective 01/21/2026, S1 stated she will immediately resolve all technical issues with Pro Care System to ensure compliance with this regulation.

Based on observation, at 12:45 p.m., S1 failed to follow the Electronic Devices Policy as electronic devices were used by children under age 2. The Specialist observed an infant lying on a Boppy pillow, watching television, while other infants slept. Corrective Action: Effective: S6 stated she will inform staff that, per policy and state regulation, children under the age of 2 will not be allowed to watch television programming to ensure compliance with this regulation.

Based on record review and interviews at 12:45 p.m. on 1/21/2026. S1 failed to ensure children were supervised at all times during rest time while at the center. The Specialist observed staff eating lunch in the kitchen area while a total of 14 children, ages infants to 4 years-old, were napping in 2 separate classrooms down the hall. S3, S4, and S5 failed to actively supervise children while asleep. Corrective Action: Effective 1/21/2026 S1, she will schedule breaks for staff to ensure that a teacher is in all the classrooms.

1719 A-C.: Based on a record review on 01/13/2026, at 12:30 p.m., S1 failed to ensure within the first day, present at the center, and prior to assuming sole responsibility of any children, each staff member shall receive center-specific orientation to policies and practices. Corrective Action: Effective 1/21/2026, S1 stated she will set a 30-day calendar reminder prior to the expiration of the training to ensure compliance with this regulation.

Based on record review at 1:30 p.m., S6 failed to have documentation that 1 of 6 staff members on the premises and accessible to children have a current certification in Pediatric First Aid and CPR before assuming sole responsibility for any children. S2 (DOH unknown) failed to have the current certification. The Specialist observed S2 alone during naptime with 11 children age 2-3 years old. Corrective Action: Effective: 01/21/2026 S6 stated she ensures all new hires have CPR and Pediatric First Aid before assuming responsibility of children, to ensure compliance with this regulation.

C: Based on the record review at 10:00 a.m., S1 failed to obtain a CCCBC-based determination of eligibility for childcare purposes from the Department for independent contractors before being present or performing services at the center when children are present. The Specialist observed that O1 (Speech Therapist) signed the visitors log for 2/21/2025, 3/7/2025, 3/21/2025, and 3/28/2025, but failed to have eligible CCCBC documentation before being present at the center or performing services.
