Based on record review on 01/21/2026, at 12:30 p.m., the center's Daily Attendance Children’s record shall be maintained to accurately include first and last name, arrival and departure times, date, and the last name of the person or entity to whom the child is released. To accurately reflect children on the center’s premises at any given time and be used to sign in and out if a child leaves and returns to the center during the day.       Based on observation at 2:30 p.m., S6 failed to have a record of all staff members and owners, including the first and last name of the staff member or owner, date, and arrival and departure times, to accurately reflect the staff members and owners on the premises at any given time. S6 did not have access to files because Procare was not working. Corrective Action: Effective 01/21/2026, S1 stated she will immediately resolve all technical issues with Pro Care System to ensure compliance with this regulation.

The Specialist observed an infant lying on a Boppy pillow, watching television, while other infants slept. Corrective Action: Effective: S6 stated she will inform staff that, per policy and state regulation, children under the age of 2 will not be allowed to watch television programming to ensure compliance with this regulation.

The Specialist observed staff eating lunch in the kitchen area while a total of 14 children, ages infants to 4 years-old, were napping in 2 separate classrooms down the hall. S3, S4, and S5 failed to actively supervise children while asleep. Corrective Action: Effective 1/21/2026 S1, she will schedule breaks for staff to ensure that a teacher is in all the classrooms.
