The facility did not have documentation to show child care personnel had begun the introductory training within 90 days of employment in the child care industry. CCF Handbook, Section 4.1 Provider had an employee listed on the supplemental page that did not start a DCF required class within 90 days of employment; however, before the inspection was over the employee did start a required training. Technical Assistance given from their handbook: 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child Child Care Facility Handbook Page | 40 care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. Supervision of children in the [the three year old classroom] group was inadequate in that [the teacher walked the children out of her classroom going outside and left one child behind in the classroom with the door shut]. CCF Handbook, Section 2.4 General Requirements [SR] On the day this happened the teacher left a child alone in the classroom with the door shut while she walked the class to the double doors that lead out to the playground. The child was heard screaming and crying by other staff members and the child was taken to another teacher in the building who comforted the child and cleaned the child up. Technical Assistance given: 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location

An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Physical Environment [SR] The child who is three years old was in line to use the bathroom and another child was able to slam his finger into the door because the door locks were broken at the time of the incident according to multiple staff. At the time of the complaint the locks on the bathroom doors were fixed. Technical Assistance given: 3.1 General Health and Safety Requirements A. All childcare facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months.

Children in the outdoor play area were not adequately supervised in that [child Z.M. received some type of injury to the back of his neck and no staff could say how the child got the injury.]. CCF Handbook, Section 2.4.1, A General Requirements [SR] The facility is being cited for this standard due to there were teachers present during outdoor play and no teachers could explain how the child sustained some type of injury. This was corrected as the child is no longer enrolled at the facility. 2.4 Supervision 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group. D. At all times, lighting must allow child care personnel to see and supervise children while in care.

Documentation of an accident or incident was insufficient in that it did not include the [parents signature]. CCF Handbook, Section 6.4, C Health Requirements [SR] Documentation of an incident was not documented after the facility was made aware that the child sustained some type of injury. This was corrected as the child does not attend the childcare facility. However, the facility is receiving a technical assistant on how to complete incident reports writing due to several parents that where interviewed stated they have not received incidents reports. 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/childcare personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. A ratio of [1] child care personnel for [6] children is required. A ratio of [2] child care personnel for [13] children was observed. s.402.305(4), F.S. General Requirements [SR] On 11/01/2023 classroom 1C which are the one-year-olds the ratio was 12 ones and 2 teachers, but then a staff member brought her elementary student into the classroom with her for the day and that put that classroom over ratio.
