Based on record review and interview at 11:00 a.m., S1 failed to maintain documentation of a daily attendance record for all staff and owners, to include their time of arrival and departure. The was no attendance records present for S1, Director (DOH 02/27/2023), S2, (DOH 02/27/2023) or S3, (DOH 02/27/2023). S1 was not aware that Administration staff needed to maintain attendance records.

S1 failed to have documentation of the time, list of children and staff that were present. S1 stated was not aware that documentation of children and staff attendance were required for non-vehicular excursions.
