      Based on record review and interview at 11:00 a.m., S1 failed to maintain documentation of a daily attendance record for all staff and owners, to include their time of arrival and departure. The was no attendance records present for S1, Director (DOH 02/27/2023), S2, (DOH 02/27/2023) or S3, (DOH 02/27/2023). S1 was not aware that Administration staff needed to maintain attendance records.

S1 failed to have documentation of training for S5 (DOH 08/23/2024). S1 stated S5 will have proof of orientation training by 02/28/2025.

      : Based on record review/interview at 11:00 a.m., S1 failed to have documentation of tornado drills that were conducted at least once per month during the months of March, April, May, and June. There was no documentation that tornado drills had been conducted within the 2024 licensing year.

      Non-vehicular Excursions Records: Based on record review/interview at 11:00 a.m., S1 failed to have documentation of non-vehicular excursions conducted at the center. On 06/19/2024, a non-vehicular excursions occurred at the center with a Dinosaur Experience which required children to leave the licensed area of the center. S1 failed to have documentation of the time, list of children and staff that were present. S1 stated was not aware that documentation of children and staff attendance were required for non-vehicular excursions.
