Based on record review on 11:45 a.m., S1 failed to have documentation that 1 of 9 staff, S3 completed the required annual Online Child Abuse and Neglect Mandated Reporter Training provided by DCFS. The Online Child Abuse and Neglect Mandated Reporter Training for S3 expired on 05/13/2024. Corrective Action: Effective 7/24/2025, S1 stated she will have staff complete the required training at the beginning of each new year to ensure compliance with the regulation.

      1901.G.H. Based on observation at 10:50 a.m., S2 failed to ensure all of the center's equipment used by children was maintained in a clean and safe condition and in good repair. The Specialists observed the play yard to have a broken plastic, red slide, and multi colored plastic playset with broken red steps. The items were removed during the visit. Corrective Action: Effective 6/18/2025 S2 stated she will remove any broken play yard equipment and have staff check the play yard equipment daily before children go outside to ensure compliance with this regulation.

Suspension or expulsions should only be considered as a final action after the implementation of behavior support strategies, including at a minimum:i. engaging parents by written communication and/or parent conference; and ii. providing a referral to EarlySteps, Child Search, and/or mental health consultant if appropriate.
