      1713.E.&F.: Based on record review at 12 p.m., while supervising a group of children, center staff failed to devote their time to the supervision of the children, meeting the needs of the children, and in participation with the children in their activities. The center’s video on 5/7/2026, showed S7 and S8 on the center’s playground and at approximately 3 p.m., C1, 1-year-old, fell off of a tricycle, causing him to injure his wrist. On 5/7/2026, at approximately 3 p.m., C1 fell off of a tricycle while on the playground, causing him tofracture his wrist. The parent was not notified of the incident.       Based on record review and interview at 12:00 p.m., S1 failed to provide documentation of tornado drills that were conducted at least once per month during the months of March, April, 2026. There were no tornado drills completed in 2026. The last tornado drill was completed in June 2025. Corrective Action: Effective 5/21/2026, S2 stated the center will conduct monthly tornado drills in May and June 2026 to ensure compliance with this regulation.

      Based on record review/interview at 12:00 p.m., S2 failed to ensure all staff members completed the Mandated Reporter training annually. 1 of 11 staff members, S5, did not have current certification. S5s Mandated Reported expired on 08/08/2025. S2 stated S5 will have proof of Mandated Reporter Training by 10/17/2025. Corrective Action: Effective 10/08/2025, S2 stated all staff will complete the Mandated Reporting Training immediately following their hire date and annually going forward, to ensure compliance with this regulation.

      1103.A.4.5.C.1 Based on interviews at 2:30 p.m., S1 failed to notify the Department within 24 hours, of the following critical incident: On 8/4/2025, S1 was made aware of an allegation by DCFS that S2 used a prohibited method of punishment, on an unknown date and time, with an unknown 2-years-old being punished for defecating on herself and made to sit in the soiled clothing. S1 did not notify the Department. Corrective Action: Effective August 7, 2025, S1 stated she will report all allegations of abuse to the appropriate agencies to ensure compliance with this regulation.

On 8/04/2025, a DCFS Staff member visited the center, but the Visitor's Log does not accurately reflect her being present. Corrective Action: Effective August 7, 2025, S1 stated she will ensure that the staff at the end of the day check the Visitors Log for accuracy, to ensure compliance with this regulation.
