Based on record review/interview on 11:25 a.m., S15 failed to supervise children at all times. On 04/13/2026, at 2:17 p.m., C1 (2-years-old) was left outside unattended on the playground for approximately 30 seconds. The Specialist observed video footage of C1 leaving the line of children and walking into the playhouse as the class walked into the center. Corrective Action: Effective 4/20/2026, S2 stated they will continue to implement counting children at each checkpoint/ barrier and have assigned additional staff with the toddler class on the playground, and for one staff…
