1911.I.&J.: Based on record review and interviews at 12 p.m., S18 failed to lift C1, age 1-year-old, using proper lifting techniques. On 4/27/2026, at 8:55 a.m., S18 picked C1 up by the back of his shorts to move him from the door, she then picked him up by his right arm and placed him onto the floor. During this time, she was observed picking him up off the floor by the front of his shirt and placing him on the sofa. Corrective Action:

The Specialist observed S4 and S5, alone, supervising 7 children ages, one to two-years-old. Based on record review/interviews at 1:43 p.m. although mandated reporters, S7 delayed the report of an allegation or suspicion of abuse/neglect of a child to the Child Protection Statewide Hotline: On Friday, 5/16/2025, at 10 a.m., S7 reported to S1 and S2 that she heard S4 hit C1, age one-year-old on the hand while on the play yard. This was reported two days later on Wednesday, 5/14/2025 at 5:00 p.m. Corrective Action: S1 stated she will retrain all staff to report incidents or allegations immediately to ensure compliance with this regulation.

S19 first day present at the center, working with children was on 5/13/2025 S1 stated S19 will complete the training by 6/6/2025. Corrective Action: Effective 6/2/2025, S1 stated she will have all new hires complete the required orientation within 3 days of hire to ensure compliance with this regulation.

S1 stated staff will complete training during winter break.

Based on record review at 1:57 p.m., S1 failed to have documentation that the entire center and play yard is checked after the last child departs to ensure that no child is left unattended at the center for the months of: August 2024, October 2024, November 4-6, 8, 11-13, 15, 16-17, 19-22, 2024 and December 2-6, 9-13, 16-18, 2024.

Based on record review at 1:39 p.m., S1 failed to have documentation of incidents, injuries, accidents, illnesses, and unusual behavior with the time of notification and time on the incident for the following days: -On 10/16/2024 a child bumped his head with another child resulting in a black eye. There was no time of parental notification.-On 10/29/2024 a child tripped and hurt their nose. There was no time of the incident.-On 11/20/2024 a child was walking a slide with a toy that scraped their forehead. There was no time of parental notification.

Based on record review at 11:00 a.m., S1 failed to immediately notify the Parent, of the following critical incident: On 10/23/2024 at 11:45 a.m., S3 forcefully grabbed C1s arm and wrist, and aggressively yelled at C1. O1 was notified on 10/23/2024 at 3:30 p.m. LDOE and DCFS were notified on 10/23/2024.

Based on interviews at 12:00 p.m., although the center had a written behavior management policy implemented, S3 (DOH 7/31/23) used a prohibited method of discipline as C1, age 2-years-old, was subject to physical discipline. On 10/23/2024 at 11:45 a.m., S3 forcefully grabbed C1s arm and wrist, and aggressively yelled at C1. S3 was terminated on 11/1/2024.

Based on record review at 9:45 a.m., S1 failed to have a CCCBC-based determination of eligibility for child care purposes from the department obtained for O6, O10, O11, and O12 prior to the person being present at the center or performing services and failed to have documentation of the paid, adult staff member not otherwise counted in child to staff ratios who accompanied O6, O10, O11, and O12 at all times while on the center premises.
