      Based on record review, LPA observed that 0 of the 6 children reviewed had not completed a medical assessment. All children reviewed were missing the LIC 701 Physician's Report in their file. Child #1 and Child #5 were also missing an immunization record on file. the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.       Based on observation of the yard, LPA observed there to be a gate that was open to an area off limits area. The gate does is currently unable to close properly. LPA also observed two long white tables that broken in multiple areas and a broken spiderman chair desk. Per licensee, the facility has not been tested for lead. Licensee stated that the children do not drink from the drinking fountains and only drink bottled water that they bring from home. Licensee did stated that she uses one of the sinks for boiling water when cooking. POC Due Date: 12/08/2023
