The food preparation area does not have a hand washing station or the hand washing station did not include the following: [bottles were being prepared in the infant room where there is only one sink for diaper changing.]. CCF Handbook, Section 3.9.1, A.7. The director said they are waiting on a part to come in to add a second sink (through the plumbing) and splash guard to use for preparing bottles. The part should arrive by 1/30/26. The director stated that they will prepare the bottles in the kitchen area until the second sink and splash guard are added in the infant room. Technical support was provided. A. A food preparation area is required for facilities that choose to prepare food in a manner consistent with the definition of preparation of food. D. Situations or times that children, child care personnel, and/or volunteers must perform hand hygiene must be posted in all food preparation, diapering, and toileting areas. Due Date 2/20/2026

Child care personnel, including volunteers who work 10 hours or more per month did not complete the required in-service training during the states fiscal year beginning July 1 and ending June 30. CCF Handbook, Section 4.2.7 The CCP listed on the Supplemental Sheet did not complete ten hours of In-Service Training last training year (7/1/24 - 6/30/25). She completed it on 7/8/25. She will need to complete an additional ten hours of training for the current training year ending 6/30/26. Technical support was provided. 4.2.7 Annual In-Service Training Upon successful completion of the 40-hour introductory training requirements, child care personnel must complete a minimum of 10 clock- hours or one CEU of in-service training annually during the states fiscal year beginning July 1 and ending June 30. Child care personnel not in compliance with the annual in-service training requirement described in this section must complete the remaining in-service training requirement within 30 days of the noncompliance finding by the licensing authority. The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. CCF Handbook, Section 7.1 LS reviewed ten new preschool children's records. The child listed on the Supplemental Sheet had an expired DH 680 form. It expired 9/5/25. She enrolled on 7/11/25. Technical support was provided. 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. B. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The teacher in the infant classroom made an attendance record for today and marked all four infants as present. Technical support was provided. 7.5 Daily Attendance Daily attendance of children must be taken and recorded accurately by the child care personnel, documenting the time when each child enters and departs the program. A. Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. The facility did not have documentation to show completion of a Department approved five-hour early literacy and language development course for child care personnel within 12 months of date of employment in child care industry and/or the early literacy course documentation was not uploaded in the Florida Pathways/Registry. CCF Handbook, Section 4.2.2 and 4.5, C The CCP listed on the Supplemental sheet started working on 4/30/24 and has not completed the required Literacy training. Technical support was provided. 4.2.2 Early Literacy Training Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. Proof of completion may be documented on a certificate of course completion, classroom transcript, or diploma. Child care personnel must complete one of the following: A. One of the Departments online literacy courses available on the Departments website. B. One of the Departments approved literacy courses. A list of these courses can be obtained from the Departments website. (No additional courses will be approved by the Department.) C. One college level early literacy course (for credit or non-credit) if taken within the last five years.

The facility did not have documentation to show child care personnel had begun the introductory training within 90 days of employment in the child care industry. CCF Handbook, Section 4.1 The two CCP listed on the Supplemental Sheet has been working over 90 days and has not started the DCF 40-hour training. Technical support was provided. 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. The child care program is responsible for obtaining training documentation from child care personnel.

Bottles and/or sippy cups brought from home were not labeled with childs first and last name. CCF Handbook, Section 3.9.5, B 16 sippy cups/water bottles did not have names on them. Staff placed names on all 16 sippy cups/water bottles during this inspection. Technical support was provided. B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily.

Child care personnel failed to ensure arrival and departure times were complete and accurate at the time of arrival or departure. CCF Handbook, Section 7.5, B None of the teachers had an attendance for their classroom. The master attendance/sign in/out sheet only had 19 children signed in. 21 children are present. Staff signed in the other two children and all staff made a current attendance for their classroom during this inspection. Technical support was provided. 7.5 Daily Attendance Daily attendance of children must be taken and recorded accurately by the child care personnel, documenting the time when each child enters and departs the program. Child care personnel inappropriately interacted with children in care in a manner that was aggressive, demeaning or intimidating in nature. CCF Handbook, Section 8.2, C The staff that work in this classroom are no longer employed at this child care facility. Their employment was terminated. Technical support was provided. C. Child care personnel must appropriately interact with children to foster a healthy, safe environment that will encourage the childs physical, intellectual, motor, and social development. Interactions with children that are aggressive, demeaning or intimidating in nature are strictly prohibited.

Child care personnel did not comply with the facility's written disciplinary and expulsion policies. CCF Handbook, Section 2.8, B A child was placed in a crib in the closet (with the door open) for time out. Another staff member saw the child and took her out and placed her on the floor beside her. The staff member that placed the child in the crib/closet was suspended, then terminated. Technical support was provided. B. The child care facility operators, employees, and volunteers must comply with written disciplinary and expulsion policies.

The ground cover or other protective surface under the [at the end of the slide] was not maintained. CCF Handbook, Section 3.12, D The fall zone at the end of the slide was not maintained. Staff raked mulch from other places on the playground that was not a fall zone and placed it at the end of the slide during this inspection. Technical support was also provided regarding the Daily Inspection Log. D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls.

CCF Handbook Section 4.2.1 The employee listed on the Supplemental Sheet started working in the Child Care Industry on 6/3/21 (started at this CCF on 6/6/22) and has not completed the required 40 hour Introductory Child Care Training requirement. Director provided a written statement that she will not work with children until she completes all training requirements. Technical support provided. 4.2 Training Requirements 4.2.1 Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better.
