      Based on observation at 1:45 p.m., S1 failed to have evidence of a current safety inspection for 1 of the 3 center's vehicles. The vehicle's safety inspection expired on 01/31/2026, and the center has been driving and transporting the children in February and March of 2026. S1 stated she will ensure the inspection sticker is updated by the end of the day. Corrective Action: Effective 03/10/2026, S1 stated she will place a reminder on her calendar to renew the vehicle's safety inspection sticker 30 days prior to expiration to ensure compliance with this regulation.

      1509.A.8.a. b.: Based on record review and observations on 07/17/2025, at 9:45 a.m., although the developed and implemented a written behavior management policy S13, DOH: 03/11/2025, used a prohibited method of discipline as C1, eight-year-old, was subjected to verbal abuse and threats of a prohibited actions. The Specialist observed video footage that was posted to S13's Instagram of S13 yelling, at C1 as he asked her to stop. Based on record review and interview on 07/17/2025, at 11:00 a.m., S1 failed to maintain the following information for O4, independent contractor, observed on the visitors sign in log on 7/16/2025: person's name, address or phone number. S1 contacted O4 prior to the specialist's departure, to retrieve the missing documents. Corrective Action: Effective 08/01/2025, S1 has created folders for all Independent Contractors, to ensure compliance with this regulation.

S7 (DOH 7/14/2025), was observed with sole responsibility of twelve, 4-5 year-old, children. Corrective Action: Effective 08/01/2025, S7 has completed certification in pediatric first aid and CPR.

Based on observation and interviews on 07/17/2025, at 10:00 a.m., S5 and S6 failed to provide written authorization from a physician for C2, 3-month-old, to sleep in a different sleeping position. The Specialist observed C2, sleeping in a crib on her stomach. S1 placed C2 on her back prior to specialist's departure. Corrective Action: Effective 08/01/2025, S1 stated she will re-train all staff on infant room regulations and guidelines(safe sleep), to ensure compliance with this regulation.
