1727.A.B. Based on record review at 12:18 p.m., S1 failed to have documentation that 2 of 8 staff, completed the annual online child abuse and neglect Mandated Reporter Training provided by DCFS. S5s certification expired on 07/10/2025 and S6s certification expired on 07/27/2024. Corrective Action: Effective 07/30/2025, S1 stated she will set a reminder 30 days prior to expiration for required yearly trainings to ensure compliance with this regulation.

1901.C. Based on record review at 12:40 p.m., S1 failed to have documentation that the entire center and play yard is checked after the last child departs to ensure that no child is left unattended at the center. There was no documentation that a visual check was conducted for the following dates: March 12, 2025, April 14, 2025, April 29, 2025 and June 24, 2025. S2 stated the center was open on those days. Corrective Action: Effective 07/30/2025, S1 stated she will conduct visual checks daily and ensure she signs off on them to ensure compliance to the regulation.

Based on observations on 07/30/2025 at 9:17 a.m., S1 failed to ensure all of the center equipment used by children was maintained in good repair. The Specialists observed a toy worm tunnel with ridged edges on the outside playground that is accessible to children. Corrective Action: Effective 07/30/2025, S1 stated she will remove any broken yard equipment and have staff check the play yard equipment before children go outside to ensure compliance with this regulation.

1911.G. Based on observations at 9:06 a.m., the Specialists observed a pacifier attached by a ribbon, to the clothing of one child, age 1-year-old. S5 removed the clip attached to the childs clothing during the licensing visit. Corrective Action: Effective 07/30/2025 S1 stated she will remind staff to not clip pacifiers to infants clothing to ensure compliance with this regulation.

1921.E. Based on record review at 10:58 a.m., S1 failed to have documentation of tornado drills that were conducted at least once per month during the month of May and June of 2025. Corrective Action: Effective 07/30/2025 S1 stated he will completed the required drills at the beginning of each month to ensure compliance with this regulation.
