Children in the outdoor play area were not adequately supervised in that [a child was left behind]. CCF Handbook, Section 2.4.1, A On March 20, 2025, a child snuck away from a teacher as the teacher was retrieving another child and was left on the playground hidden from view. The child knocked on a window of another classroom and the teacher escorted him to his classroom. One of the teachers in the 2-year-old classroom was terminated on 3/21/2025. The Child Care Facility handbook states on page 12: 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised.

Child care personnel failed to share the accident/incident form with the custodial parent/ legal guardian or the individual authorized to pick up child on the day that the incident occurred. CCF Handbook, Section 6.4, B & D Specialist interviewed the teacher that witnessed the incident February 20th, 2025. She was busy and forgot to notify the parent or have the parent sign the accident/incident report when the child was picked up. When the parent came to pick the child on the last day of enrollment, they asked for all accident/incident reports involving their child. That was when it was discovered that the parent was not notified. The parents signed the incident report once it was discovered. The Child Care Facility Handbook states on pages 55 and 56: B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form.

Child care personnel failed to immediately contact emergency services and the child's custodial parents or legal guardians in the event of a serious illness, accident, injury or emergency to their child. CCF Handbook, Section 6.3, B & C and 2.5.3, E A child had what appeared to be a medical emergency, as witnessed on video. The teachers did not report to the administration or parents. The two teachers were terminated. The Child Care Facility Handbook states on page 55: B. For life threatening injuries, the provider shall call 911 and then notify the parent. C. Custodial parents or legal guardians must be notified immediately if the event of any serious illness, accident, injury or emergency to their child, and their specific instructions regarding action to be taken under such circumstances must be obtained and followed. The Child Care Facility Handbook states on page 16: E. Child care personnel shall notify parents/guardians immediately of any suspected allergic reactions, as well as the ingestion of or contact with the problem allergen even if a reaction did not occur. Child care personnel shall contact 911 immediately whenever epinephrine has been administered.

Supervision of children in the [1 to 2 year old ] group was inadequate in that [a 14 month old child escaped classroom and staff were unaware . ]. CCF Handbook, Section 2.4 On 6/10/24 a 14-month-old child escaped the classroom and proceeded to walk down the hallway inside facility. Classroom staff where unaware child had left as their view was blocked by a parent who had opened door. Child was stopped before entering lobby of facility by Administrative Assistant and brought back to classroom. 2.4 Supervision page 12 2.4.1 General Supervision Requirements B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location.

The facility did not have documented proof that all child care personnel were trained and knowledgeable within 30 days of date of hire in [ safe sleep practices and shaken baby syndrome]. CCF Handbook, Section 4.2.3 and 4.2.5 2 facility caregivers did not have required safe sleep training within 30-day time frame of industry start date. 4.2.3 Safe Sleep/ Shaken Baby Syndrome Training page 41 All child care personnel who work in a facility that offers care to infants must have training regarding guidance on safe sleep practices, preventing shaken baby syndrome and abusive head trauma; recognition of signs and symptoms of shaken baby syndrome and abusive head trauma; strategies for coping with crying, fussing, or distraught child and the development and vulnerabilities of the brain in infancy in early childhood within 30 days of hire at the facility. For child care personnel to satisfy this requirement the training must be accomplished through one of following methods: Department's Health Safety and Nutrition course, Safe Sleep course, or the Early Learning Floridas Safe Sleep Practices. Documentation of training must be maintained on the Department's training transcript in the child care personnel record.

Supervision of children in the [two year old] group was inadequate in that [a child was left in the classroom bathroom when the class left to the playground.]. CCF Handbook, Section 2.4 A child was left in the classroom bathroom when the class went to the playground. Another teacher needed to enter this classroom for something and found the child. The child was brought to her group on the playground. The CCF Handbook states that childcare personnel must be assigned to provide direct supervision to a specific group of children at all times. Children must never be left without childcare personnel supervision inside or outside the facility.

The child was left on the playground for about 10 minutes until another class entered the playground and the child was brought back to his room. The CCF Handbook states that childcare personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. Children must never be left without childcare personnel supervision inside or outside of the facility.

The documentation must be shared with the custodial parent or legal guardian on the date of occurrence. Documentation must include signatures of program child care personnel and the custodial parent or legal guardian.

Supervision of children in the [2 year old classroom] group was inadequate in that [a child was brought into the classroom from the playground and was left in the classroom when the staff member was distracted by another staff member. The CCF Handbook states that]. CCF Handbook, Section 2.4 A child was left alone in a classroom when she was brought inside from the playground to use the bathroom. The CCF Handbook states that childcare personnel must be assigned to provide direct supervision to a specific group of children and be with that group at all times.

Child care personnel including volunteers who work 10 hours or more per month did not complete the 40 hour Introductory Child Care Training requirement. CCF Handbook Section 4.2.1 The staff member noted on the supplemental form has not completed the required 45 hours of training. This staff member began in the industry 7/26/2021. Training needed to be complete by 10/18/2022. The CCF Handbook states that child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department.

The facility did not have documentation to show child care personnel had begun the introductory training within 90 days of employment in the child care industry. CCF Handbook, Section 4.1 The 2 staff members noted on the supplemental form did not begin the required training within 90 days of employment. The CCF Handbook states that child care personnel must begin training within 90 days of employment in the child care industry.
