      The facility did not have documentation to show child care personnel had begun the introductory training within 90 days of employment in the child care industry. CCF Handbook, Section 4.1 One staff member started working in industry and facility on 4/23/25. She did complete Safe Sleep training within 30 days of employment, but this was under School Readiness. Required DCF training was not started within 90 days of employment as required. 4.1 Beginning Training page 39-40 Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination.       Children in the outdoor play area were not adequately supervised in that [as a child was able to go in and out of classroom twice without out being seen by staff in outside play space. ]. CCF Handbook, Section 2.4.1, A On 9/8/25, a child was able to slip in and out of classroom twice without direct supervision from staff responsible for him. This was reported by a parent watching live feed of classroom. This was evident as video observed verified child did go in and out of classroom from outside play space. Staff member responsible for child was terminated. A mandatory staff meeting was held to address direct supervision of children. All current staff received a 1-hour in-service certificate for training on 9/24/25. 2.4 Supervision page 12 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times.       Child care personnel failed to document an accident or incident on the day that it occurred. CCF Handbook, Section 6.4, A A 2-year-old child left facility with a limp on 4.22.25. It was discovered during investigation that a virus had caused child to limp and have swelling on knee. Child was uncomfortable and not bearing weight at times on her leg between 4.22.24 and 4.24.25. Incidents reports were dated 4.22.25, 4.23.25 and 4.24.25 but were not generated until 4.28.25 and signed by parent on 4.30.25. 6.4 Accident/Incident Notification page 55 and 56 A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian.       Supervision of children in the [2 year old group ] group was inadequate in that [1 two year old was left unattended in outside playspace for 4 minutes. ]. CCF Handbook, Section 2.4 On 9/25/24 at approximately 4:30pm, a 2-year-old child was left unattended in outside play space by 2 staff. This was self-reported by facility. Both staff received written warning and retraining. CCF Handbook, Section 2.4 On 9/23/24, Specialist observed video dated 9/20/24 of aggressive and hurtful behavior from one child to another. 2 staff that were responsible were engaged in an activity that did not focus on children's actions or behaviors in classroom. Staff members involved were reprimanded and given written warnings. Emergency service telephone numbers must be posted on or near all telephones, including 911, ambulance, fire, police, poison control center, Florida Abuse Hotline, and the county public health department. Additionally, the address and directions to the facility must be posted with descriptions of major intersections and local landmarks.
