      Based on record review at 9:45 a.m., S1 failed to have documentation that 1 of 7 staff, S3 (Date of Hire: 02/27/2023), on the premises and accessible to the children had current certification in Infant, Child, and Adult CPR through training approved by the Department. S3 CPR certification expired on 04/30/2025 and she completed training on 5/9/2025. Corrective Action: Effective 07/22/2025, S1 stated she will schedule trainings prior to the expiration date, to ensure compliance with this regulation.

      Based on observation at 11 a.m., S1 failed to ensure foods implicated in choking incidents were not served to children under age 4. Specialist observed chunks of meat (sausages), larger than what can be swallowed whole, and whole grapes were served to twenty-one, 1-to-3-year-old, children. This was corrected during the inspection.
