Gingerbread House Creative Learning Center II

Based on record review at 11:00 a.m., S1 failed to obtain an explanation from the parent when something was observed upon a child's arrival at the center. The specialist observed observations dated 04/01/2026 to present and the explanation from the parent or child failed to be documented. This was not corrected during the inspection. Corrective Action: Effective 05/18/2026, S1 stated that all staff will be retrained on observation documentation, to ensure compliance with this regulation.

Based on record review at 10:00 a.m., S1 failed to have documentation on file for O2, Independent Contractor, that included the person's name, address, phone number, and list of duties performed while at the center. O2 was present in the childcare center on 9/08/2025 and 9/15/2025. Corrective Action: Effective 09/23/2025, S1 stated she will attach a note and additional forms to the visitor daily log for all contractors to complete at their next visit, to ensure compliance with this regulation.

Based on record review/interviews on 3/21/2025, at 9:45 a.m., S1 failed to immediately notify parents of the following critical incident: On 3/18/2025, at an unknown time, C1 (3-years-old), C2 (3-years-old), C3 (3-years-old), C4 (2-years-old), and C5 (2-years-old) exited the playground without any staff noticing and wondered to a grassy field two streets over from the center, for an unknown amount of time. O6 and O7 returned the children to the center at approximately 11:36 a.m. The parents were contacted beginning after 1:09 p.m.

Based on record review/interviews on 3/21/2025, at 10:00 a.m., S2, S3, and S4 failed to ensure children were supervision at all times on 3/18/2025. C1 (3-years-old), C2 (3-years-old), C3 (3-years-old), C4 (2-years-old), and C5 (2-years-old) exited the playground without any staff noticing and wondered to a grassy field two streets over from the center, for an unknown amount of time. O6 and O7 returned the children to the center at approximately 11:36 a.m.

Based on observations/record review on 3/21/2025, at 10:15 a.m., S1 failed to ensure the outdoor play space was enclosure with a permanent barrier in a manner that prevents children from leaving the premises without proper supervision as on 3/18/2025, C1 (3-years-old), C2 (3-years-old), C3 (3-years-old), C4 (2-years-old), and C5 (2-years-old) exited the playground without any staff noticing and wondered to a grassy field two streets over from the center, for an unknown amount of time.

Based on record review at 10:30 a.m., S1 failed to provide documentation that 1 of 4 staff, S5, received the required orientation within seven days of the first day present at the center on 12/23/2024. S5 completed the DCFS Mandated Reporter Training on 1/17/2025 and the LDE Key Training Module on 1/15/2025.

Based on record review/interview at 1 p.m., the medication administration record for C5, 5-years-old, signed and dated by O1 on 12/23/2024, for Methylphenidate to be administered at 12:30 p.m., failed to include the date the medication was to be administered and the medication side effects and/or anticipated reactions. The authorization failed to include the date, time, and dosage of the medication administered, and S7's signature, the staff who administered the medication. This was not corrected during the inspection.
