Suggestions/Recommendations: Angie needs to put an activity permission/consent form in all enrolled children's file and have parent sign. This form needs to be updated annually with other enrollment forms. Angie does not take children anywhere and didn't believe this form was required, but will place it in all kids' files. Corrective Action Required: A follow up visit will not be required. Child files will be checked again next year at her annual compliance visit.
