      1713.A.&B.&C.: Based on record review and interviews at 11:00 a.m., S17 failed to ensure children were supervised at all times. On 12/02/2025, at 1:03 p.m., C1, three-years-old, was left unsupervised in the classroom for approximately five minutes. S17 was supervising fifteen 3-year-olds, during naptime, when the fire alarm sounded for a fire drill at 1:02 p.m. S17 gathered the children and went outside leaving C1 on her mat asleep at 1:03 p.m. At 1:08 p.m., S1 observed C1 walking out of the building and walking up to her. Based on record review/interview(s) at 11:30 a.m., S1 failed to notify the parents immediately, and the Department within 24 hours, of the following critical incident: On 11/27/2024, at 2:44 p.m., S9 and O4 observed S21 using profanity towards three children, C1, one-year-old, C2, one-year-old, and C3, one-year-old. The Department was notified of the incident the next business day, 12/02/2024, at 3:29 p.m. Parents were also notified on 12/02/2024. S1 stated the reports were delayed due to her conducting an internal investigation.
