Based on record review and interview at 2:15 p.m., S1 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the Department for each staff member, S4, prior to the person being present at the center or performing services. S4's first day working at the center is 05/28/2026. S1 stated she thought S4 already had an eligible CCCBC.
