1711.A.D.1. Based on observation at 10:39 a.m., S1 failed to meet the required child to staff ratio for infants. The Specialist observed S2, alone, with 6 children, ages 4 to 11-months-old. The required ratio for infants is five children per 1 staff person. One additional staff was needed to satisfy child to staff ratio. Ratio was corrected at 11:04 a.m. when S5 entered the classroom. Corrective Action: Effective 9/30/2025, S1 states she will revise staff schedules for enough staff coverage to ensure compliance with this regulation.

Based on record review at 10:15 a.m., S1, who is qualified as a Director, did not have documentation of being on-site full time(32 hours weekly) at the qualified Director at the center during the day time hours of operation (prior to 9:00 p.m.) and responsible for planning, managing, and controlling the center's daily activities, as well as responding to parental concerns and ensuring that minimum licensing requirements are met. Based on observations at 10:49 a.m., the indoor and outdoor area were not free of hazards as the following:-C1, age two-years-old was able to grab a pair of sharp scissors that were laying on the childrens round table in the two year old classroom. -The outdoor area had containers with sitting water accessible to children, two deflated jumper houses accessible to children, a water hose pipe laying unraveled on the ground accessible to children, and random trash and debris in the outdoor play area accessible to the children. 1903. D.5 Based on observations/interview at 10:46 a.m., the Specialists observed the number of children using a room was exceeded based on the 35 square feet per child requirement. The one year old room is licensed to accommodate 5 children and 7 were present. S1 moved two children into the two year old classroom with S4 and S7. Corrective Action: Effective 6/9/2025, S1 stated she will place a dry erase board for room capacity number inside each classroom to ensure compliance with this regulation.

1903.E.5. Based on observation at 10:50 a.m., S1 failed to have the outdoor play space enclosure with a permanent fence or other permanent barrier in a manner that protects children from traffic hazards, prevents children from leaving the premises without proper supervision, and prevents contact with animals or unauthorized persons. The outdoor play space fence did not have a secure latch and the bottom pole was broken that prevents the gate from closing securely. 1919.H. Based on observations at 10:41 a.m., the Specialists observed C2, age six-months-old laying in a crib on her back holding and drinking from her bottle. Corrective Action: Effective 6/9/2025, S1 stated all infants will receive their bottle while sitting up on the mat or held by a staff to ensure compliance with this regulation.

1921.E. Based on record review at 12:43 p.m., S1 failed to have documentation of tornado drills that were conducted at least once per month during the months of March, April, and May 2025. Date of last tornado drills were March, April, May, and June 2024. Corrective Action: Effective 6/9/2025, S1 stated she will set an alarm for the appropriate months to completed the required drills.

Based on record review at 11:58 a.m., S1 failed to have documentation for 1 of 5 staff on the premises and accessible to children have current certification in infant, child, and adult CPR through training approved by the Department. S6 (DOH 1/25/2023) failed to have the current certification. A class has been scheduled for 12/19/2024.

Based on record review at 11:18 a.m., S1 failed to have documentation of a CCCBC-based determination of eligibility available for S3 (DOH 12/7/2023) conducted at least once during a five-year period. S3s CCCBC expired on 12/6/2024. S3 was present at the center 12/6/2024, 12/9/2024, 12/10/2024, 12/11/2024, 12/12/2024, 12/13/2024, and 12/17/2024. S3 has not become eligible for childcare purposes. S3 left off premises at 11:23 a.m.
