Based on record review at 11:45 a.m. S2 failed to have documentation that S2 (DOH 5/01/1987), S1(DOH 7/30/2018), S3 (DOH 5/09/2019), S4 (DOH 11/02/2022), S5 (DOH 09/28/2015) and S6 (DOH 6/26/2024) completed the yearly DCFS online Mandated Reporter Training prior to having sole responsibility for any children. Corrective Action: Effective 5/1/2025, S2 stated she will have staff complete the required training immediately to ensure compliance with this regulation.

Based on observations at 10:15 a.m., S2 failed to prohibit the use of tobacco substances on the childcare premises as Specialists observed a trash can filled with cigarette buds located in the laundry room trashcan as well as cigarette ashes and multiple cigarette on the floor of the laundry room. Specialist also observed a half smoked cigarette in the laundry room located on a shelf. Corrective Action: Effective 05/01/2025, S2 stated she will have all staff relocate away from the daycare facility when smoking cigarettes.

1919.B.2 Based on observations at 10:25 a.m., S2 failed to have menu substitutions or additions posted, written or electronically, on or near the menus as the Specialists observed the children served chicken nuggets, green beans, and milk. The posted menu read the lunch would be baked bbq chicken, whole wheat bread, mac and cheese, oranges, sliced cucumbers and milk. Corrective Action: Effective 5/01/2025 S2 stated she will make sure that the food menu is posted at all times with substitutions to ensure compliance with this regulation.
