      Based on observation and interviews at 9:37 a.m., S1 failed to ensure the outdoor play space had a permanent fence or other permanent barrier that prevents children from leaving the premises without proper supervision, and prevents contact with animals or unauthorized persons. The Specialist observed part of the center’s fence down due to a tree falling. S4 stated the fence has been down approximately 2 weeks and S1 has been trying to get the fence repaired. The Specialist observed S1 preparing lunch while supervising 9 three to four-year-old children. S1 stated there was no additional staff available to cook and prepare lunch at the center. Corrective Action: Effective 05/07/2026, S1 stated she will adjust staff schedules to ensure enough staff are present to supervise each classroom so S1 can prepare lunch to ensure compliance with this regulation.
