Based on record review and interviews at 10:49 a.m., S1 failed to have documentation of children's records kept a minimum of two years from the date of termination of enrollment at the center for C1. C1's last day in the center was 07/01/2025. Corrective Action: Effective 05/13/2026, S1 stated she will keep former students records in a folder for 3 years to ensure compliance with the regulation.

1713.B. Based on interviews at 10:49 a.m., S5 failed to ensure that children were supervised at all times. On 4/30/2026, time unknown, C2, age 2-years-old, was found by O2, unattended in a highchair, in the lobby while S5 walked outside to her car. Corrective Action: Effective 05/13/2026, S1 stated she will remind staff daily and conduct a quarterly meeting about active supervision to ensure compliance with this regulation.

1919.B.2 Based on observations on at 12:10 p.m., S1 failed to have menu substitutions or additions posted, written or electronically, on or near the menu. The Specialist observed the children eating beef stew with mixed vegetables, apple sauce and milk. The menu posted stated the lunch for the day would be fish sticks, mashed potatoes with cheese, peas, bread, apples and milk. Corrective Action: Effective 05/13/2026, S1 stated she will check daily for proper modification of the menu and place a sticky note for that day to ensure compliance with the regulation.

Based on record review and interviews at 11:00 a.m. on 6/30/2025, S1 and S2 failed to provide required documentation of current commercial liability insurance for the operation of the center to ensure medical coverage for children in the event of accident or injury. The Specialist was provided with with insurance documentation with coverage dates from 3/1/2024-3/1/2025, however, when the Specialist called the insurance agent to verify the policy, it was stated the center never had a policy with Ozark Insurance Agency and the policy number provided was for Little Dreamers Learning…

Based on record review at 10 a.m. on 6/30/2025, S2 failed to post a current 'The Safety Box' newsletter issued by the Louisiana Office of the Attorney General. The Safety Box posted expired in December 2024. Corrective Action: Effective 6/30/2025, S2 stated she would post The Safety Box by the end of today, and going forward, she will check the website for the updated Safety Box every 3 months and post it to ensure compliance with this regulation.

Based on observations at 10:00 a.m. on 6/30/2025, S2 failed to post the current weekly menu listing specific food items served for each day of the week by the first day of each week and remain posted throughout the week. The menu posted in the center was not current. Corrective Action: Effective 6/30/2025, S2 stated she would post a menu by the end of today, and going forward, she will make sure the menu is posted at the beginning of each week to ensure compliance with this regulation.
