S3 stated on 01/23/2026, C1, age 1-yearold, had dark marks under his eyes upon arrival. Corrective Action: Effective 2/4/2026, S1 stated she will check the daily observation logs at the beginning of each day to ensure compliance with this regulation.

C.: Based on record review/interview on 5/19/2025, at 3:00 p.m., S1 failed to notify the department within 24 hours of the following critical incident: On 5/1/2025, at 11:45 a.m., S4 reported that S14 plucked C1 on his head, while on the playground, because he told S4 to shut up. The department was notified on 5/2/2025, at 3:46 p.m. The incident was reported to Child Welfare on 5/1/2025, at 3:55 p.m. Corrective Action: Effective 5/19/2025, S1 stated he will make notify all required departments. following a critical incident, immediately to ensure compliance with this regulation.

C1 (two-years-old), told S4 to shut up on 5/1/2025, around 11:45 a.m., and S14 plucked him on his head. C1 placed his hand near his face, and began to cry. S1 reported the incident to the Department and Child Welfare on 2/04/2025. The Specialist was unable to validate the allegation.
