      Based on observation, the licensee did not comply with the section cited above as LPA observed the bathroom in between class rooms # 3 & #4 had a toilet with a broken seat cover. LPA also observed carpets, rugs, and rubber floor tiles with multiple stains in classroom #1 through #3 which poses/posed a potential health, safety or personal rights risk to persons in care.
