A.: Based on record review, at 9 a.m., the children's daily attendance for children failed to accurately reflect the children on the childcare premises at given time as 36 children were present and 31 children were signed in on the record. A review of the children's attendance from 01/02/2025 to present failed to include the time of departure of each child on 8 times and the first and last name of person or entity to whom the child was released 1 time.       Based on record review at 9:45 a.m., 2 of 15 staff, S5 and S13, failed to complete the online child abuse and neglect Mandated Reporter Training provided by DCFS annually in 2024. Corrective Action: Effective 03/20/2025, S1 stated she will have staff complete mandated reporter training at the same time yearly , to ensure compliance with this regulation.

      Based on record review/interview at 11 a.m., S15 and S16 failed to supervise C1, 1-year-old, at all times. On 11/26/2024, at 4 p.m., S15 (Date of Hire: 9/19/2024) and S16 (11/21/2024) left C1 on the side play yard after they entered the building. O2 observed C1 from the parking lot and alerted S3, who then went to C1 to bring inside. C1 was unsupervised on the play yard for approximately four minutes. S15 was terminated on 11/26/2024 and S16 was terminated on 12/5/2024.
